1. Who we are

evictionforms.io (“we,” “us”) provides self-help document preparation software that helps New York landlords assemble notices such as the 14-Day Rent Demand, the 5-Day Late Notice, and 30/60/90-Day Termination Notices.

2. Not a law firm; no legal advice

We are not a law firm, we are not your attorney, and we do not provide legal advice. Using this service does not create an attorney-client relationship. Our software organizes the information you supply into a document format and calculates dates based on the answers you give. It does not review your situation, determine whether a notice is appropriate, or guarantee any outcome in court. Eviction law, service requirements, and local rules vary by county and by court. You should consult a licensed New York attorney before serving or filing any document.

3. Your responsibility for accuracy

You are solely responsible for the accuracy and completeness of every fact you enter, including names, addresses, amounts owed, rental periods, occupancy start dates, and service dates. We generate documents from your inputs without independent verification. You are also responsible for serving the notice correctly and for keeping proof of service.

4. Payment

• Notices are $150 all-in for one tenant — the document, certified mail with return receipt, and personal service by a process server — or included in a monthly plan as described on our pricing page. Each additional adult tenant named on the notice must be served and mailed separately and adds $100.
• Court filing packets are priced separately, as shown on our pricing page.
• You can preview a watermarked draft for free. Payment removes the watermark and unlocks the final document.
• Subscription plans renew automatically until cancelled. You can cancel at any time; cancellation takes effect at the end of the current billing period.
• Prices are shown in U.S. dollars. Applicable taxes are added at checkout.

6. Acceptable use

You agree not to use the service to harass any person, to prepare a notice you know to be false or retaliatory, to attempt a self-help lockout or other illegal eviction, or to resell, scrape, or automate access to the service without our written permission.

7. Availability and changes

We may update forms, calculations, and features as New York law and court forms change. We do our best to keep the service available but do not guarantee uninterrupted access.

8. Limitation of liability

To the maximum extent permitted by law, our total liability for any claim relating to the service is limited to the amount you paid us for the document at issue. We are not liable for indirect, incidental, or consequential damages, including lost rent, dismissed proceedings, or delays.

9. Governing law

These terms are governed by the laws of the State of New York, without regard to conflict of law rules.
